1. Why Placental Cord Insertion Matters

The umbilical cord normally delivers two umbilical arteries and one umbilical vein into the placental chorionic plate, where branching vessels distribute blood flow across the placental disk. In a typical central or paracentral/eccentric placental cord insertion, Wharton jelly protects the vessels until they reach the placental surface. The key ultrasound question is therefore anatomical: are the vessels protected and rooted in the placental parenchyma, or are they exposed within the membranes before reaching the placenta?

The International Society of Ultrasound in Obstetrics and Gynecology notes that formal assessment of placental cord insertion is not universally required as part of the routine mid-trimester scan; however, marginal or velamentous cord insertions should be reported when visualized (Salomon et al., 2022). Commonly cited prevalence patterns include central insertion in approximately 80% of pregnancies, paracentral insertion in approximately 12%, marginal insertion within 2 cm of the placental edge in approximately 5% to 8%, and velamentous insertion in approximately 1% (Salomon et al., 2022).

Do not overcall. Do not undercall.

Overcalling an abnormal placental cord insertion may create anxiety and unnecessary follow-up. Undercalling a velamentous or lower-edge insertion may miss exposed fetal vessels and the need to exclude vasa previa. The sonographer’s role is to document the anatomy clearly, demonstrate the vessel course when possible, and communicate uncertainty honestly.

Clinical significance

The risk spectrum is not the same for all insertion patterns. Central and paracentral/eccentric insertions are generally considered normal variants. Marginal insertion has been associated with adverse perinatal outcomes, including small-for-gestational-age neonates and preterm delivery; however, management remains variable, and not every marginal insertion is clinically consequential (Siargkas et al., 2023a). Velamentous insertion carries greater concern because fetal vessels travel through the membranes without Wharton jelly protection before reaching the placenta. This finding has been associated with fetal growth restriction and vasa previa risk, particularly when exposed vessels are located near the lower uterine segment or cervix (Buchanan-Hughes et al., 2020; Salomon et al., 2022; Siargkas et al., 2023b). Furcate insertion is rare but important because the cord vessels separate before placental insertion and lose Wharton jelly protection, which may predispose the vessels to rupture, thrombosis, or fetal compromise in reported cases (Xu et al., 2024).

2. Core Terminology and Diagnostic Criteria

Terminology pitfalls

Students should not confuse the fetal abdominal cord insertion with the placental cord insertion. The fetal abdominal insertion is assessed for ventral wall defects, whereas the placental cord insertion is assessed at the placental disk, membranes, and chorionic plate.

A cord loop lying near the placental edge should not automatically be called a marginal insertion. The sonographer must follow the cord into the placental tissue and confirm the true root. Likewise, velamentous insertion should not be diagnosed unless the cord inserts into the membranes and the vessels are shown coursing unprotected before entering the placenta (Nomiyama et al., 1998; Sepulveda et al., 2003). A low velamentous insertion, bilobed placenta, succenturiate lobe, resolved previa, or vessels near the cervix should trigger vasa previa screening logic rather than stopping at the placental cord insertion label (Society for Maternal-Fetal Medicine, 2015/2016).

3. Sonographic Features by Insertion Type

Central insertion

A central placental cord insertion is usually the easiest pattern to document. The cord enters the broad central region of the placental disk, often with a perpendicular or near-perpendicular root into the chorionic plate. On gray-scale imaging, the cord is surrounded by echogenic Wharton jelly until it reaches the placental surface. Color Doppler can show the umbilical arteries and vein entering the disk and branching across the chorionic plate.

The best view is a broad image of the placental disk with the cord root centered in the field. Documentation may include the three-vessel cord when part of the exam protocol, the insertion site, and any limitations. A common student trap is assuming an insertion is central based on a partial placenta image; if the whole disk is not shown, the insertion may actually be eccentric.

Paracentral / eccentric insertion

Paracentral or eccentric insertion is off-center but still comfortably within the placental disk. It is commonly defined as a lateral insertion more than 2 cm from the placental edge. This is generally treated as a normal variant, but the sonographer should still prove that the cord enters the placental parenchyma and is not simply draped along the surface.

If the insertion is near the edge, the shortest distance from the placental margin to the insertion root should be measured according to local protocol. Color Doppler can help separate true cord vessels from chorionic plate vessels. A clear report may state: “Placental cord insertion is eccentric, approximately ___ cm from the nearest placental margin.”

Marginal insertion/battledore placenta

Marginal insertion describes cord entry at or near the placental edge. A commonly used threshold is within 2 cm of the placental margin, although definitions vary across studies and clinical protocols (Liu et al., 2002; Salomon et al., 2022). The term “battledore placenta” is an older descriptive term referring to a placenta with the cord attached at the margin, similar to a battledore or racket handle.

On gray-scale imaging, the cord reaches the thin peripheral placental tissue rather than the central disk. On color Doppler, vessels enter the chorionic plate at the edge rather than traveling across free membranes before reaching the disk. The sonographer should sweep around the root in a 360-degree fashion and demonstrate that the vessels are entering placental parenchyma. The shortest distance from the cord insertion to the placental edge should be documented. If the root is flush with the edge, it may be described as marginal or edge insertion.

Marginal does not equal velamentous

In marginal insertion, the cord still inserts into placental tissue at the edge. In velamentous insertion, the cord inserts into the membranes, and the vessels travel without Wharton's jelly before reaching the placenta.

Velamentous insertion

Velamentous insertion is a high-yield diagnosis for ultrasound students because it is fundamentally a vessel-protection problem. The umbilical cord inserts into the membranes rather than directly into the placental disk. The umbilical vessels then travel along the membranes before entering the placental parenchyma. ISUOG describes velamentous insertion as insertion of the umbilical vessels within the amniotic membranes instead of the placenta and notes its association with vasa previa and fetal growth restriction (Salomon et al., 2022).

On gray-scale imaging, the main cord trunk may appear to terminate outside the placental margin or may fail to dive directly into the disk. On color Doppler, the examiner may see fan-like divergence of red and blue vessels over the membranes before they reach the placental edge. A helpful sign is vessel fixation: membrane-bound vessels remain in place with maternal repositioning or gentle probe movement, unlike a free cord loop.

If exposed vessels are near the lower uterine segment, cervix, low-lying placenta, or accessory lobe, the next step is targeted vasa previa evaluation using color Doppler and transvaginal ultrasound when indicated (Nomiyama et al., 1998; Society for Maternal-Fetal Medicine, 2015/2016).

Furcate insertion

Furcate insertion is rare but belongs in a complete teaching chapter because the term may appear in pathology reports, case discussions, and high-level ultrasound teaching. In furcate insertion, the cord vessels separate before reaching the placenta and lose Wharton jelly protection before they insert. Unlike velamentous insertion, the separated vessels may still enter the placental parenchyma; the key abnormality is premature separation and loss of the protective cord sheath.

The gray-scale clue is that the cord trunk loses its common sheath before reaching the placental surface. Color Doppler may show individual vessels forking, running parallel, spiraling, or entering tissue or membranes independently. Because the evidence base is limited and largely based on case reports and reviews, the sonographer should describe the finding anatomically and recommend physician or maternal-fetal medicine review rather than overpromising management guidance (Smith et al., 2022; Xu et al., 2024).

4. Ultrasound Scanning Protocol for PCI Assessment

Step 1: Find the Placenta Before Chasing the Cord

Begin with placental location, edge visibility, relationship to the cervix, and whether the placenta appears single-lobed, bilobed, or succenturiate. The placental map determines how much effort is needed to prove the placental cord insertion, especially when the lower uterine segment is involved.

Step 2: Identify the Cord Root on Gray-Scale

Use a wide enough field of view to see the placental edge and the cord approach. Zoom only after the larger anatomy is understood. Follow the cord from the amniotic cavity to the placenta; do not accept a tangential surface loop as the insertion. In central, eccentric, and marginal patterns, look for Wharton jelly continuing to the placental surface.

Step 3: Add Color or Power Doppler

Color Doppler helps trace umbilical vessels and distinguish true insertion vessels from chorionic plate vessels or maternal vessels. Power Doppler may be useful when flow is slow or the vessel angle is challenging. Doppler should be optimized with a focused color box, adequate gain, and a pulse repetition frequency low enough to detect slower venous or membrane vessel flow without excessive artifact (Nomiyama et al., 1998; Sepulveda et al., 2003).

For the best technique, narrow the color box to the insertion region to preserve frame rate. Lower the PRF or scale when small membrane vessels or venous flow are suspected. Increase color gain until light background noise appears, then decrease slightly. Use power Doppler for sensitivity when direction is less important than vessel detection.

Step 4: Sweep 360 Degrees Around the Insertion

A true marginal insertion should show cord vessels entering the placental parenchyma at the edge. A suspected velamentous insertion should show the cord ending in membranes, with vessels traveling before entering the disk. Sweeping around the insertion protects students from the classic error of confusing a nearby free cord loop for the placental cord insertion.

Step 5: Measure and Document

When the root is near the placental edge, measure the shortest distance between the insertion and the placental margin. Document placental location and whether the lower edge or cervix was evaluated. If the placental cord insertion is not visualized, state the reason, such as fetal position, posterior placenta, maternal habitus, contractions, acoustic shadowing, or late gestational age. If vessels are seen near the cervix, move from routine placental cord insertion documentation to a vasa previa screening protocol.

5. Lower Uterine Segment Triage and Vasa Previa Link

The Society for Maternal-Fetal Medicine defines vasa previa as unprotected umbilical vessels running through the amniotic membranes and passing over the cervix. Type I is classically associated with velamentous cord insertion. Type II is associated with succenturiate or multilobed/bilobed placenta, where connecting fetal vessels cross over or near the internal cervical os (Society for Maternal-Fetal Medicine, 2015/2016). Therefore, the sonographer who identifies a low-edge velamentous insertion, bilobed placenta, succenturiate lobe, resolved previa, or suspicious vessels in the lower uterine segment should not stop at the placental cord insertion label. The next question must be: where are those fetal vessels relative to the internal cervical os?

Targeted transvaginal color Doppler should be used when the placental edge, suspected velamentous vessels, connecting vessels between lobes, or unexplained linear vascular structures are near the cervix or lower uterine segment. Spectral Doppler can help confirm fetal versus maternal circulation.

6. Documentation and Reporting Language

Clear reporting is especially important because placental cord insertion terminology can be used inconsistently across departments. A useful report does not only name the insertion type; it also explains the evidence: location, distance to margin, Doppler findings, relationship to the cervix, and any limitations (Ward et al., 2023, 2024).

Minimum image set for student competency

A strong student image set should include a gray-scale image showing the cord approach and placental edge or disk; a color or power Doppler image showing vessel entry into placental tissue or exposed vessels in membranes; a measurement image if the insertion is near the placental edge; and a cervical or lower uterine segment image if the placenta is low, bilobed, succenturiate, or if velamentous vessels are near the cervix. An optional cine sweep can be helpful when classification is uncertain.

What the Literature Supports

Placental cord insertion visualization is easiest earlier in gestation, particularly around the detailed anatomy scan, and becomes more difficult with advancing gestational age, fetal crowding, maternal habitus, and posterior placental location (Salomon et al., 2022; Ward et al., 2024). ISUOG recommends reporting marginal or velamentous insertion when visualized and notes that velamentous insertion may be associated with vasa previa and fetal growth restriction (Salomon et al., 2022). Color and power Doppler improve the ability to trace vessel course, particularly for suspected velamentous insertion or lower uterine segment vessels (Nomiyama et al., 1998; Sepulveda et al., 2003). Marginal insertion has been associated with adverse outcomes in meta-analysis, but not all marginal insertions behave the same clinically; distance from the placental edge, placental location, fetal growth, and local protocol matter (Siargkas et al., 2023a). Furcate insertion has limited evidence, and most management statements are extrapolated from case reports or small case series rather than large prospective studies (Smith et al., 2022; Xu et al., 2024).
